Entrance Requirements

• Original High School/SHS Report Card
• Certificate of Good Moral Character
• Form 137 / Transcript of Records
• Birth Certificate (PSA)
• Honorable Dismissal (if transferee)
• Pictures: 3 (1x1), 2 (2x2), 5 Passport size
• Police & NBI Clearance
• Medical Exam Report
